Aplicación de hipotermia terapéutica para prevenir el deterioro cognitivo tras una parada cardiorrespiratoria.

Abstract

Objetivo: Analizar la información existente en la literatura sobre el beneficio de la hipotermia terapéutica para prevenir el deterioro cognitivo tras una parada cardiorrespiratoria. Método: Se ha realizado una revisión sistemática siguiendo la metodología PRISMA. Empleando las bases de datos PubMed y Web of Science se encontraron 223 trabajos de los cuales 4 cumplían los criterios de inclusión. Se llevó a cabo una lectura sistemática de los trabajos seleccionados con el fin de extraer la información necesaria para evaluar las diferencias en relación con la prevalencia de daño cognitivo entre los pacientes a los que se les ha aplicado hipotermia terapéutica frente a aquellos a los que no; analizar los dominios cognitivos más afectados en los pacientes con parada cardiorrespiratoria y los instrumentos de evaluación utilizados; y comprobar si se ha aplicado rehabilitación cognitiva en estos pacientes. Resultados y discusión: No existe ningún estudio que haya comparado el estado cognitivo de pacientes con parada cardiorrespiratoria que han sido sometidos o no a hipotermia terapéutica. Los estudios incluidos en la presente revisión sólo comparan diferentes temperaturas objetivo en el contexto de la aplicación de la hipotermia terapéutica. Por lo tanto, no se puede estimar la prevalencia exacta de las alteraciones cognitivas. No obstante, parece que en los pacientes con parada cardiorrespiratoria que son tratados con hipotermia terapéutica, la prevalencia de las alteraciones cognitivas se encuentra en torno al 50%. La función cognitiva que parece alterarse con mayor frecuencia es la memoria. Conclusión: No existen estudios publicados que demuestren que la aplicación de hipotermia terapéutica en pacientes con parada cardiorrespiratoria protege frente al desarrollo de alteraciones cognitivas.

Objective: To analyze the existing information in the literature on the benefit of therapeutic hypothermia to prevent cognitive impairment after cardiorespiratory arrest. Methods: A systematic review has been carried out following the PRISMA methodology. Using the PubMed and Web of Science databases, 223 works were found, of which 4 met the inclusion criteria. A systematic reading of the selected works was carried out in order to extract the necessary information to evaluate the differences in relation to the prevalence of cognitive impairment between the patients to whom therapeutic hypothermia has been applied and those who have not; to analyze the cognitive domains most affected in patients with cardiorespiratory arrest and the assessment instruments used; and check if cognitive rehabilitation has been applied in these patients. Results and discussion: There is no study that has compared the cognitive status of patients with cardiorespiratory arrest who have or have not undergone therapeutic hypothermia. The studies included in the present review only compare different target temperatures in the context of the application of therapeutic hypothermia. Therefore, the exact prevalence of cognitive alterations cannot be estimated. Notwithstanding, it appears that in patients with cardiorespiratory arrest who are treated with therapeutic hypothermia, the prevalence of cognitive alterations is around 50%. The cognitive function that seems to be altered most frequently is memory. Conclusion: There are no published studies showing that the application of therapeutic hypothermia in patients with cardiorespiratory arrest protects against the development of cognitive alterations.
