El papel de la enfermería en la detección de la hidrocefalia normotensiva idiopática (síndrome de Hakim-Adams)

La Hidrocefalia Normotensiva Idiopática (HNTi) o Síndrome de Hakim-Adams, se
manifiesta mediante una triada clínica característica que incluye alteración de la marcha,
incontinencia urinaria y un deterioro cognitivo progresivo. En la actualidad, su tratamiento consiste
en la implantación de un sistema de derivación ventriculoperitoneal.
Debido al riesgo de la cirugía y las características de los pacientes, resulta esencial realizar
un diagnóstico temprano y correcto del síndrome, de manera que los pacientes obtengan un mayor
beneficio del tratamiento.
Dentro de los equipos sanitarios, es el personal de enfermería el que emplea más tiempo en
el cuidado y atención del paciente, por ello, serían estos los primeros en detectar la aparición de los
síntomas y signos de la enfermedad.
La valoración de los pacientes que sugieran HNTi se realizará mediante una batería de tests
que definirán el perfil de deterioro que presenta el paciente.
Así mismo, esta valoración permitirá detectar la enfermedad de manera precoz o la
coexistencia de otras causas de demencia y determinar la eficacia del tratamiento quirúrgico.
Por lo tanto, el papel de la enfermería resulta fundamental para la evaluación temprana de
las esferas clínica, funcional y neuropsicológica de los pacientes, contribuyendo así al diagnóstico
final de la HNTi. Idiopathic Normal-Pressure Hydrocephalus (INPH) or Hakim-Adams Syndrome consists of
a clinical triad that includes gait disturbance, urinary incontinence, and cognitive impairment.
The treatment consists of an implantation of a ventriculoperitoneal shunt system. Due to the
risk of surgery and patient characteristics, it is essential to make an early and correct diagnosis of the
syndrome, so the patients get the most benefit possible from the treatment.
The nurses are who spend the most time in the care and attention of the patient, therefore,
they would be the first to detect the appearance of symptoms and signs of the disease.
The assessment of patients suggesting INPH should be carried out by means of a battery of
tests that will define the deterioration profile of the patient.
This assessment will also make possible to detect the disease early or to identify other causes
of dementia and to determine the efficacy of surgical treatment. Therefore, the role of nursing is
fundamental for the early evaluation of patients' clinical, functional, and neuropsychological spheres,
thus contributing to the final diagnosis of INPH.
