Estudio de la muerte súbita de origen cardiovascular de 0-50 años y del total de muertes naturales y violentas de 2015 a 2019 en el IML de Tenerife

2020Abstract
INTRODUCCIÓN. Tanto la muerte súbita, de etiología natural, como la muerte violenta, son
objeto de estudio en los Institutos Médico-Legales. Ambas representan un grave problema de
salud pública. Como principal causa de muerte en el mundo, la cardiovascular también
prevalece en lo que a muertes súbitas se refiere, siendo las coronariopatías causantes de entre
el 80 y el 85% de las mismas. OBJETIVOS. Relacionar la muerte súbita cardíaca de 0-50 años
con relaciones clínico-analíticas para su diagnóstico precoz. Hallar tóxicos presentes en la
autopsia en cardiovasculares y suicidios y hallar variaciones de masa encefálica y cardíaca entre
grupos, por sexo, edad y causa fundamental. Realizar un estudio global médico-legal por edad
y sexo. Analizar el total de autopsias de 2015-2019. MATERIAL Y MÉTODOS. Estudio
epidemiológico, observacional, retrospectivo y descriptivo, en el que se estudió el total de
autopsias realizadas en la Isla de Tenerife entre 2015-2019 (primer grupo) y específicamente,
los cardiovasculares de 0-50 años y SMSL entre 2015-2017 (segundo grupo). RESULTADOS.
Las muertes de causa cardiaca supusieron el 68,6% de todas las muertes de causa natural del
primer grupo, siendo la cardiopatía isquémica junto a la fibrosis miocárdica sin especificar el
29% de las muertes totales tanto naturales como violentas, entre los años 2015-2017 y el
78,11% de las muertes cardiacas de ese periodo. Mayor frecuencia en varones en ambas series
estudiadas (72% vs.28% en el total; 81% vs 19% en cardiovasculares de 0-50). Las muertes
naturales representan el 50,9% del total, las violentas (46,2%) (siendo en esta última la causa
principal las accidentales), e indeterminadas (2,87%). Existe diferencia significativa entre la
masa del encéfalo y corazón entre sexos de 0-50 años. Respectivamente: varones (1453,4 cm3

s.d. 130,3) (p-valor = 0,001). Se observa una ligera correlación positiva entre edad y
peso cardíaco y negativa entre edad y peso encefálico. Sí es significativa (p-valor<0,001) la
correlación entre tamaño del corazón y dimensión del encéfalo (r = 0,621). Se detectaron
parámetros clínicos de interés en las muertes cardiovasculares de 0-50 años en al menos, el 30%
de los casos. Se detectaron tóxicos en el 55% de este grupo (25% desconocidos, 20% negativos),
siendo los más frecuentes las benzodiacepinas/barbitúricos (35%) y la presencia de THC (31%).
Por su parte, en los suicidios se hallaron tóxicos en al menos, el 46,5% de los casos (45,6%
desconocidos, 8% negativos). Las benzodiacepinas/barbitúricos, estuvieron presentes en el
58% de los positivos. CONCLUSIÓN. Existe una distribución irregular por edad y sexo,
siendo más frecuentes los casos de hombres y las edades de 50-70 años. En causas
cardiovasculares, la más frecuente es el grupo “cardiopatía isquémica/fibrosis miocárdica”. El

grupo de BZD/barbitúricos y el cannabis, al igual que los signos clínicos premórtem fueron
descritos en, al menos, un tercio de los pacientes. Existe variación entre la masa encefálica y
cardíaca respecto a la edad y entre sexos, variando la masa cardíaca claramente entre distintas
causas cardiacas. Las etiologías violentas más frecuentes son las accidentales, seguida de las
suicidas y por último las homicidas, delatando en suicidas un alto uso de fármacos
psicotrópicos. INTRODUCTION. Sudden deaths, natural in etiology, just like violent deaths, are the object
of study in the Medical-Legal Institutes. Both represent a serious public health problem. As the
main cause of death in the world, cardiovascular disease also prevails in terms of sudden deaths,
with coronary artery disease causing between 80 and 85% of them. OBJECTIVES. To relate
sudden cardiac death aged 0-50 years with clinical-analytical parameters for its early diagnosis.
To perform a frequency analysis by sex, age and main causes groups. To find intergroup
variations in brain and cardiac mass. To classify all deaths according to medicolegal etiology.
To find toxins present at autopsy in young cardiovascular deaths and suicides. To analyze total
death causes from 2015 to 2019. MATERIAL AND METHODS. Epidemiological,
observational, retrospective and descriptive study, in which the total autopsies carried out in
Tenerife from 2015 to 2019 (first group) and specifically, cardiovascular and SIDS in people
aged 0-50, between 2015-2017 (second group), were studied. RESULTS. The most common
cause was the cardiac deaths group (68,6% of all natural causes in the first group) being the
ischemic heart disease/unspecified myocardial fibrosis cases 29% of all global deaths from
2015 to 2017 and 78,11% of all cardiac deaths in this period. There was a higher frequency in
men in both series (72% vs. 28% in the total; 81% vs 19% in cardiovascular 0-50.) Natural
deaths represent 50,9% of the total, violent deaths (46,2%), undetermined (2,87%). Within the
violent deaths group, the main causes were accidental deaths. There is a significant difference
between the mass of the brain and heart in young cardiovascular group: men (1453.4 cm3 sd
114.3) vs. women (1298.3 cm3 sd 91.3), (men 476.4 cm3 sd 128.2 and women 380.6 cm3 sd
130.3) (p-value = 0.001). A slight positive correlation between age and cardiac weight and a
negative correlation between age and brain weight were observed, without being statistically
significant (p-value = 0.08). The correlation between heart size and brain size is significant (p-

value <0.001) (r = 0.621). Clinical parameters of interest were detected in cardiovascular deaths
aged 0-50, in 30% of the cases. Drugs and toxins found in them were detected in 55% in this
group. The most frequent are benzodiazepines/barbiturates (35%) and the presence of THC
(31%). On the other hand, suicides were found under substances in 46.4% of the cases.
Benzodiazepines or barbiturates were present in 58% of the positives. CONCLUSION. There
is an irregular distribution by age and sex, with the most frequent cases of men and the ages of
50-70 years. In cardiovascular causes, the most frequent is ischemic heart disease together with
myocardial fibrosis of any causes. Benzodiazepines/barbiturates group and cannabis, as well as
the premortem clinical signs were described in almost a third of the patients in the
cardiovascular group who were under toxics. There is variation between brain and cardiac mass
with respect to age and between sexes, with cardiac mass varying clearly between different
cardiac causes. The most frequent violent etiologies are accidental, followed by suicidal and
finally homicidal, revealing in suicides a high use of psychotropic drugs.
