Aplicación de crioterapia contra crema anestésica EMLA® para el manejo del dolor por punción en pacientes portadores de fístula arterio-venosa interna: un ensayo clínico cruzado

La prevalencia de Insuficiencia Renal Crónica ha aumentado en los últimos años
y, con ella, el uso de tratamientos renales sustitutivos como la hemodiálisis. Para este
tratamiento, es necesario un acceso venoso de gran calibre, siendo la fístula arterio-venosa
interna (FAVI) el de primera elección. Del mismo modo, los pacientes que reciben
hemodiálisis a través de este acceso se ven sometidos a numerosas punciones al mes,
generándoles dolor y ansiedad al momento del tratamiento y, con ello, el empeoramiento
de su calidad de vida.
En cuanto a esto, se ha visto necesario encontrar un método para el alivio de este
dolor que sea efectivo y accesible para todos los pacientes. Mejorando así la situación de
ansiedad que genera a los pacientes el momento de la punción.
El método más usado para el alivio de este dolor en los últimos años ha sido la
crema anestésica EMLA®
, la cual ha demostrado ser un método efectivo, pero con un coste
elevado y con la posible aparición de efectos adversos por su uso prolongado. Por otro
lado, en numerosos estudios se ha considerado la crioterapia como un método igualmente
válido y con menos efectos negativos sobre el paciente.
Con este estudio, se pretende comprobar si la crioterapia puede ser una buena
alternativa ante la crema EMLA®
, consiguiendo un mayor alivio del dolor en el momento de
la punción.
Para ello, se aplicará ambos métodos a los pacientes portadores de FAVI que
reciben su tratamiento en el Hospital Universitario de La Palma, comparando las dos
técnicas y evaluando cual ha resultado ser el más efectivo. Prevalence of Chronic Renal Failure has raised in the past recent years and with

an high caliber vennous access being the arteriovenous fistula our first choice. Patients who
are being treated with hemodialysis through this access suffer from a lot of punctures by
the month, getting a lot of anxiety and pain while the treatment is being administered
causing a bad quality of life for our patients.
It is necessary to find a better method for our patients to relieve the pain caused
by the treatment. An effective method and accessible for all patients improving the anxiety
caused by the treatment when the punction is being done.
The most popular method for the pain relief in the past years has been the
anesthetic cream EMLA® which has been an effective method but it's really expensive and
has some harmful effects if you use it for a long time. But in numerous papers, cryotherapy
has been considered an effective method too with less negative effects on the patient.
In this essay I will try to verify if cryotherapy can be a good alternative for EMLA®
cream, getting a bigger pain relief when the punction is being done. I will apply different
methods to the patients who get their treatment at the Hospital Universitario de La Palma
comparing both methods and evaluating which one has been more effective.
