Herramientas de enfermería en el manejo del delirium.

INTRODUCCIÓN. El delirio es una alteración grave de la atención, la orientación y la
conciencia, de etiología multifactorial y de base orgánica; prevenible y reversible. Es un
síndrome aún desconocido, en términos fisiopatológicos, y su importancia reside en su alta
incidencia, asociada a un mal pronóstico. OBJETIVOS. Identificar los recursos existentes para
Enfermería en la identificación y el manejo del delirio en el paciente hospitalizado, así como
enumerar las herramientas de diagnóstico, definir las intervenciones enfermeras y la
importancia de estos profesionales en su manejo. METODOLOGÍA. Se realiza una revisión
bibliográfica de las bases de datos en línea MedlinePlus, Scielo, Cuiden y PubMed, y el
recurso de libre acceso Google Académico. RESULTADOS. El recurso más eficaz para hacer
frente al delirio es la prevención, y la estrategia no farmacológica es el tratamiento de elección.
Un Plan de Actuación enfermero incide positivamente en el pronóstico y evolución del cuadro,
y basa sus intervenciones en: la monitorización del estado cognitivo y de la orientación, el
fomento de la actividad física y el descanso, la adecuación de una buena conducta alimentaria
y un correcto estado nutricional, la adaptación de un entorno apropiado y la educación a la
familia. DISCUSIÓN y CONCLUSIONES. Dada la especificidad no farmacológica de las
actividades propuestas por la bibliografía, Enfermería se encuentra en una situación
aventajada para el manejo del delirio. Esto justifica el adiestramiento de las enfermeras en
herramientas específicas en prevención e identificación, como la escala CAM-ICU y el Manual
DSM-V, y su especialización en intervenciones de la taxonomía NNN. INTRODUCTION. Delirium is a serious alteration of attention, orientation and consciousness,
of multifactorial and organic-based etiology; preventable and reversible. It is a syndrome still
unknown, in pathophysiological terms, and its importance lies in its high incidence, associated
with a poor prognosis. OBJECTIVES. Identify existing resources for Nursing in the
identification and management of delirium in the hospitalized patient, as well as list diagnostic
tools, define nursing interventions and the importance of these professionals in their
management. METHODOLOGY. A bibliographic review of the online databases MedlinePlus,
Scielo, Cuiden and PubMed, and the open access resource Google Scholar. RESULTS. The
most effective remedy for coping with delirium is prevention, and the non-pharmacological
strategy is treatment of choice. A Nursing Action Plan positively affects the prognosis and
evolution of the picture, and bases its interventions on: the monitoring of cognitive status and
orientation, the promotion of physical activity and rest, the adequacy of good eating behaviour
and proper nutritional status, the adaptation of an appropriate environment and education to
the family. DISCUSSION and CONCLUSIONS. Given the non-pharmacological specificity of
the activities proposed by the literature, Nursing is in an advantageous situation for the
management of delirium. This justifies the training of nurses in specific tools in prevention and
identification, such as the CAM-ICU scale and the DSM-V Manual, and their specialization in
NNN taxonomy interventions.
