When this occurs, a differentialdiagnosis should be carried out between ‘lymphocyte spillover’from the thymus into the peripheral blood and an associatedT-cell chronic lymphocytic leukaemia [1]. The etiology of thisphenomenon is uncertain and may reflect the perturbation ofsystemic immunoregulation that accompanies some thymicneoplasia [2].We describe a new case of a metastastic lymphocyte-richthymoma and T-cell lymphocytosis not responding toantitumour therapy.
