Seguimiento farmacoterapéutico de pacientes polimedicados

Pharmaceutical care includes many pharmaceuticals services, such health education, patient counseling, pharmacy referral service as Personalised Medication Dosage Systems, doctor-pharmacist comunication and Pharmaceutical Care. In this progam, the community pharmacist collaborates with a multidisciplinary health teamwork and a patient in monitoring a therapeutic plan through several interviews, where there is a fluid comunication between the patient and the pharmacist. The final objetive is to improve
the patient’s life quality. Pharmaceutical Care program is recomended for polymedicated patients that have more than five drugs prescibed by the doctor. The pharmacist has to be able to find inappropriate prescribing, noncompliance with prescribed medication and adverse drug reactions or interactions. This research is based on a Pharmaceutical Care study group of 16 polymedicated patients, where 13 of them needed health education, 6 were assigned to Personalised
Medication Dosage System and 5 to Pharmaceutical Care. In 5 cases, communication doctor-pharmacist was necessary. As well, in 9 cases several actions were needfully. Likewise, 2 of the patients did not complete the study. Conclusions archieved in this work are related to bad aderence’s medication, poor health education and drug reactions benefit new diseases.
