Diseño de un programa de salidas terapéuticas para pacientes con trastorno mental severo.

Abstract

Este proyecto, pretende realizar una propuesta de intervención de enfermería, cuyo objetivo es el diseño y planificación de salidas terapéuticas mediante la rehabilitación psicosocial para los pacientes con trastorno mental severo ingresados en las URAs del Área Externa de Psiquiatría del CHUC de Santa Cruz de Tenerife. Se trabajará a través de salidas programadas fuera del Centro en base a tres áreas: habilidades de interacción social, actividades instrumentales de la vida diaria y ocio y tiempo libre. Las salidas tendrán lugar entre el mes de marzo y el mayo, siendo en total 12 semanas. El número máximo de pacientes que asume este programa será de 18 y deberán cumplir con los criterios de inclusión establecidos: estar ingresado en la URA del CHUC, estar estable psicopatológicamente, con poco o escaso apoyo familiar, y con puntuación mayor de 15 en la escala Likert de los indicadores seleccionados del NOC [0900] Cognición. En función de la puntuación obtenida en este NOC, asignaremos a los pacientes a dos grupos, el A con mayor capacidad cognitiva, que puede asumir a 12 pacientes, subdivididos en A1 y A2 con 6 pacientes cada uno y el grupo B, con menor capacidad cognitiva que puede asumir también a 6 pacientes. Las actividades serán específicas y concretas para cada grupo, y basadas en la NIC: Potenciación de la socialización [5100], Terapia de entretenimiento [5360] y Enseñanza individual [5606]. La evaluación de este programa se realizará tras la primera salida, en el epicentro y al final del programa utilizando indicadores de los NOC: Autocuidado: AIVD [0306], Habilidades de interacción social [1502] y participación en actividades de ocio [1604]. Este proyecto contará con recursos propios de la Unidad.

The aim of this project is to develop a nursing intervention proposal, whose objective is the designing and planning of therapeutic outings through psychosocial rehabilitation for patients with severe mental disorders admitted to the Active Rehabilitation Unit (ARU) of the Outpatient Psychiatric Area of the CHUC of Santa Cruz de Tenerife. Work will be done through programmed outings outside the center based on three areas: social interaction skills, instrumental activities of daily living and leisure and free time. The outings will take place between March and May, for a total of 12 weeks. The maximum number of patients to be included in this program will be 18 and they must meet the established inclusion criteria: be admitted to the ARU of the CHUC, be psychopathologically stable, with little or no family support, and with a score over 15 on the Likert scale of the selected indicators of the NOC [0900] Cognition. Depending on the score obtained in this NOC, we will assign patients to two groups, group A with greater cognitive capacity, which can take on 12 patients, subdivided into A1 and A2 with 6 patients each, and group B, with less cognitive capacity, which can also take on 6 patients. The activities will be specific for each group, and based on the NIC: Socialization enhancement [5100], Entertainment therapy [5360] and Individual teaching [5606]. The evaluation of this program will be performed after the first outing, at the epicenter and at the end of the program using indicators from the NOCs: Self-care: IADLs [0306], Social interaction skills [1502] and Participation in leisure activities [1604]. This project will be supported by the Unit's own resources.
