Utilidad del índice FEV1/FEV6 en el cribado de la EPOC

Introducción: La EPOC es una enfermedad infradiagnosticada, con alta prevalencia, y a
menudo asociada a comorbilidades. Actualmente las sociedades científicas recomiendan realizar
espirometrías en pacientes fumadores con síntomas, para mejorar el diagnóstico de la EPOC. Sin
embargo, la realización de la espirometría puede ser dificultosa por la presencia de dependencia
funcional o deterioro cognitivo. Para facilitar el cribado de la EPOC, se ha comenzado a utilizar
dispositivos que miden el índice FEV1/FEV6. No obstante, hoy en día no está implantado en
atención primaria ni está incluido dentro del Proceso de Atención Integrado de la EPOC.
Objetivo: Valorar la utilidad del uso del índice FEV1/FEV6 en pacientes con sospecha
clínica de EPOC que acudan al Área de Salud de Santa Cruz de la Palma.
Metodología: Se llevará a cabo un estudio transversal, con una muestra estimada de 202
sujetos. Aleatoriamente se les realizará la espirometría forzada o la medición del flujo espiratorio a
través un dispositivo denominado Piko 6. Se registrarán una serie de variables sociodemográficas,
y se determinará la dependencia funcional, el deterioro cognitivo y la comorbilidad de los
pacientes, objetivando los motivos que influyen en la imposibilidad de realizar las maniobras
correctas de espirometría. Tras obtener el número de pruebas útiles con ambas técnicas
espirométricas, podremos hacer un análisis estadístico comparando ambas proporciones,
determinando el nivel infradiagnóstico de la EPOC si lo hubiera, así como la utilidad o no del
dispositivo Piko 6, justificando su implantación en atención primaria. Introduction: COPD is a highly prevalent disease, underdiagnosed, and often associated
with comorbidities. The scientific community currently recommends performing spirometry in
smoking patients with symptoms to improve the diagnosis of COPD. However, performing
spirometry can be difficult due to the presence of cognitive impairment or functional dependence.
To facilitate COPD screening, portable devices that use the FEV1/FEV6 index have started being
used. However, nowadays it is not implemented in primary care nor is it included in the Integrated
COPD Care Process.
Objective: To assess the usefulness of the FEV1/FEV6 index in patients suspected of
suffering COPD in the Santa Cruz de la Palma health area.
Methodology: A cross-sectional study will be carried out, with an estimated sample of
202 subjects. Forced spirometry or expiratory flow measurement will be randomly performed
through the Piko 6 device. A series of sociodemographic variables will be recorded, and functional
dependence, cognitive impairment and comorbidity of the patients will be determined, identifying
the factors which influence the viability of performing the spirometry correctly. After obtaining the
number of useful tests with both spirometric techniques, we will be able to perform a statistical
analysis comparing both proportions, determining the underdiagnosis level of COPD, if any, as
well as the usefulness or otherwise of the Piko 6 device, justifying its implantation in primary care.
