Utilización del monitor Analgesia Nociception Index (ANI) para optimizar la analgesia en pacientes sometidos a sedación: proyecto de investigación.

2022Abstract
El abordaje del paciente con dolor constituye un reto importante en la prestación
de cuidados críticos de calidad por parte de los profesionales de enfermería,
especialmente en las/los enfermeras/os de cuidados críticos. Esto se debe a que son
pacientes que cursan con situaciones de agitación y con dolencias frecuentes, que
requieren sedación y analgesia para su tratamiento.
Los profesionales de enfermería de cuidados críticos constituyen una pieza
fundamental en la valoración y el tratamiento del paciente crítico con dolor, ya que son
capaces de detectar cualquier cambio en su estado y aplicar unos cuidados enfermeros
adecuándolos a cada situación. Las/los enfermeras/os, realizan parte de este abordaje
del dolor por medio de las escalas de valoración, que siempre tienen un componente
subjetivo. Por ello, en este proyecto se plantea la implantación del Analgesia
Nociception Index (ANI), un monitor que mide el grado de nocicepción del paciente
crítico sedado, en el que se muestra la presencia de nocicepción como un valor
numérico y objetivo.
Este proyecto se realizará en el Hospital Universitario Nuestra Señora de
Candelaria, empleando una metodología mixta: con un enfoque cuantitativo, por medio
de un cuestionario, y un enfoque cualitativo, empleando un grupo de discusión para
conocer la opinión de las enfermeras acerca de la nueva herramienta.
Esto tendrá como utilidad la integración de la herramienta de apoyo a la
valoración enfermera del paciente con dolor, para así conseguir un aumento de la
calidad de los cuidados de enfermería que se prestan a los pacientes críticos mejorando
su bienestar y control adecuado del dolor. Approaching the pain patient is a challenge in the provision of quality critical care
by nursing professionals, especially critical care nurses. This is because these are
patients with agitated situations and frequent complaints, who require sedation and
analgesia for their treatment.
Critical care nurses are an essential part of the assessment and management of
the critically ill patient with pain, as they are able to detect any change in the patient's
condition and apply appropriate nursing care to each situation. Nurses are part of this
approach to pain by means of assessment scales, which always have a subjective
component. For this reason, this project proposes the implementation of the Analgesia
Nociception Index (ANI), a monitor that measures the degree of nociception of the
sedated critical patient, in which the presence of nociception is shown as a numerical
and objective value.
This project will be carried out at the Hospital Universitario Nuestra Señora de
Candelaria, using a mixed methodology: with a quantitative perspective, by means of a
questionnaire, and a qualitative perspective, using a discussion group to find out the
nurses' opinion of the new tool.
This will be useful for the integration of the tool to support the nursing assessment
of patients with pain, in order to achieve an increase in the quality of nursing care
provided to critically ill patients, improving their well-being and adequate pain control.
