Detección de problemas relacionados con los medicamentos y resultados negativos asociados a la medicación en pacientes polimedicados

2024Abstract
Introducción: El seguimiento farmacoterapéutico destaca en el avance
de la prescripción de medicamentos, productos sanitarios y la cronificación de
patologías, correlacionados con el aumento de la esperanza de vida.
Objetivos: Mejorar la salud de los pacientes revisando sus tratamientos
farmacoterapéuticos, visibilizar las funciones de los farmacéuticos comunitarios
y estrechar la relación entre los profesionales sanitarios.
Materiales: Se utilizaron bases de datos y cuestionarios para el registro
de datos y evaluación de la adherencia farmacoterapéutica.
Metodología: Se recopilaron y evaluaron diversos planes
farmacoterapéuticos mediante entrevistas, identificando y resolviendo los
posibles PRMs y/o RNMs.
Resultados y discusión: Formaron parte del estudio 20 pacientes, con
un promedio de edad de 73±12,62 años.
Se informó de la importancia del ejercicio diario, llevar una alimentación
equilibrada e individualizada a pacientes sedentarios y/o que presentaban
padecimientos crónicos.
Al 30% (n=6) de los fumadores se les informó de los riesgos de este hábito,
terapias sustitutivas y programas de deshabituación tabáquica.
De aquellos que consumían alcohol, se facilitó el servicio de seguimiento y
derivación a colectivos al 40% (n=8).
Resalta la detección de 14 PRMs, cuyo promedio por paciente fue de 2±1
PRMs.
Respecto a los pacientes no adherentes del test de Morisky-Green, el 55%
(n=11) recibió pautas educativas y de apoyo familiar.
Conclusiones: El servicio de detección de PRMs/RNMs proporciona
educación sanitaria y, la revisión de los planes farmacoterapéuticos facilita la
derivación a atención primaria para su evaluación.
