Estudio de bridas congénitas y hernias internas en pacientes adultos

2020Abstract
Introducción. La hernia interna y la brida congénita son causas extremadamente raras de
obstrucción intestinal en adultos.
Objetivo. Determinar rasgos epidemiológicos, complicaciones pre y post operatorias, y
determinar factores predictivos de resección intestinal de pacientes con diagnóstico de
brida congénita y hernia interna.
Material y métodos. Se llevó a cabo un estudio descriptivo, retrospectivo y
observacional de los pacientes adultos del servicio de Cirugía General y Digestiva del
Hospital Universitario de Canarias que han sido intervenidos quirúrgicamente entre
noviembre de 2001 y febrero de 2020 de obstrucción intestinal secundaria a brida
congénita o hernia interna. Se excluyó a los pacientes con antecedentes personales de
laparotomías, enfermedad inflamatoria intestinal y peritonitis.
Resultados. Un total de 25 pacientes, 19 hombres y 6 mujeres, fueron intervenidos de
obstrucción intestinal por brida congénita o hernia interna. El 100% refirieron dolor
abdominal. A 22 (88%) se les realizó TAC anterior a la cirugía. En 6 pacientes fue
necesaria la resección de parte del intestino delgado debido al compromiso vascular. Se
registraron 2 exitus relacionados con la causa.
Conclusión. En este trabajo se presentan signos clínicos y radiológicos que pueden guiar
la sospecha diagnóstica precoz y permitir un tratamiento quirúrgico eficaz. Introduction. Internal hernia and congenital bands are extremely rare causes of intestinal
obstruction in adults.
Objective. To determine epidemiological features, pre- and post-operative
complications, and to determine predictive factors of intestinal resection in patients
diagnosed with congenital bands and internal hernia.
Materials and methods. A descriptive, retrospective and observational study of adult
patients at the General and Digestive Surgery Unit of Hospital Universitario de Canarias,
who underwent surgery for intestinal obstruction secondary to congenital bands or
internal hernia between November 2001 and February 2020. Patients with a personal
history of laparotomies, inflammatory bowel disease and peritonitis were excluded.
Results: A total of 25 patients, 19 men and 6 women, underwent bowel obstruction due
to congenital band or internal hernia. 100% reported abdominal pain. 22 (88%) had a CT
scan prior to surgery. Resection of part of the small intestine was necessary in 6 patients
due to vascular compromise. There were 2 deaths related to the cause.
Conclusion. This tesis shows clinical and radiological signs that can lead to early
diagnostic suspicion and allow effective surgical treatment.
